Certificate of Good Standing

REQUIRED DOCUMENTS: (If urgernt extra add 2500 Tk)

Application (stating reason) Form for Good Standing Certificate

1. Bio-Data with working experience(s) (if any);
2. place(s) and duration of work to be mentioned.
3. Character Certificate from a Registered Physician or a First Class Gazetted Officer (quoting his/her Registration Number accordingly) with respective Seal and Signature.
4. Valid BM&DC Full Registration Certificate’s photocopy.
5. Two copies of recent Passport Size photographs (must be attested on the backside by a Registered Physician/First Class Gazetted Officer).
6. Registration Number of the Registered Physician must be quoted under his/her Seal and Signature.
7. Performance Certificate from Last Employment Authority (Inland or abroad).
8. The Form “Declaration by a Medical Practitioner / Dentist” must be signed by the applicant.
9. Fees for Certificate of Good Standing must be paid via online (for Bangladeshi Doctors).
10. Those who are in Govt. Service should apply through his/her controlling officer.
11. M.B.B.S./B.D.S. Original Certificate and Photostat copy.
12. Copy of Twelve digit “Taxpayer’s Identification Number” (e-TIN Certificate with the QR Code in it) for Bangladeshi Doctors.
13. Photocopy of National Identity Card (NID Card/Smart Card).

* Photocopies must be attested by a Registered Physician/1st class Gazetted Officer with Seal, Signature and Date.

* The applicant must be submit the documents on the day of Certificate delivery.
